Job summary

Privia Health in Houston is seeking a Population Health Associate for a hybrid role requiring in-office Tuesdays and Thursdays at 1200 Binz St Suite 1490. The position supports population health initiatives and collaborates with Quality, Risk Adjustment, and Practice Operations teams to drive performance.

The role involves both virtual and on-site engagement with Privia providers, travel to PODS meetings, and occasional after-hours work to accommodate provider availability.

Qualifications

• Bachelor's degree preferred with commensurate experience.
• 3+ years in a medical office setting.
• Healthcare experience with value-based care programs (MSSP, MIPS, HEDIS, STARS) preferred.
• Fluent in EMR clinical workflows; AthenaNet experience preferred.
• Basic knowledge of coding and documentation (Hierarchical Condition Coding) preferred.
• Strong Excel skills.
• Must reside in market of assignment and comply with HIPAA.

Responsibilities

• Reviews and analyzes performance data to create actionable reports with measurable goals for access, quality, coding, documentation, and value-based care metrics.
• Present data and reports to providers and care center staff during regular meetings on site, driving accountability.
• Maintain ownership over performance metrics for each assigned care center.
• Build collaborative relationships with care centers to drive value-based care performance.
• Provide quality measure training and presentations to staff and providers as needed.
• Use EMR knowledge to guide workflows and troubleshoot to support performance.
• Analyze medical records to identify quality measure gap closure opportunities and trends.
• Submit supplemental data to payer portals.
• Participate in special projects and perform other duties as assigned.
