Job summary

Privia Health is seeking a Population Health Quality Coordinator to work on-site in Fort Worth, TX with the Market-based Population Health teams. You will perform patient outreach, coordinate care, and review performance data to drive regional action plans and target attainment.

The role requires clinical or case-management background, HIPAA awareness, and strong communication skills to support at-risk patients and care centers.

Qualifications

• Minimum of MA, EMT, or Case Management certification or clinical experience.
• Experience with telephonic or remote care coordination preferred.
• Familiarity with EHR systems and HIPAA guidelines.

Responsibilities

• Conduct patient outreach to support quality programs and meet targets.
• Assist in scheduling PCP and specialist appointments.
• Prepare reports from EMR and payer portals for teams.
• Review performance data and help create action plans.
• Build relationships with care centers and population health staff.
• Demonstrate critical thinking to support care centers.
