MedStar Health seeks a Population Health Equity Outreach Coordinator to engage attributed patients for health risk assessments, annual wellness visits, and chronic care management services. You will educate the public about programs, develop health campaigns, and support KPI-driven initiatives.

The role emphasizes data gathering, scheduling efficiency, and collaboration to improve health outcomes across diverse populations, with a focus on community outreach and health equity.

Qualifications

• High School Diploma/GED required.
• Bachelor’s degree in public health, Health Administration, or related field.
• 1–2 years of experience in population health, managed care, or quality management.
• 1–2 years of experience working with diverse populations and providing culturally competent care.
• Licensed by the State of Maryland.

Responsibilities

• Conducts outreach to Population Health patients to schedule Health Risk Assessments, Annual Wellness Visits, and Chronic Care Management services.
• Gathers and manages patient data, including demographics and medical history, while performing registration and pre-visit tasks.
• Schedules patient visits efficiently, ensuring adherence to policies and maximizing provider availability.
• Composes patient correspondence and prepares reports to support Population Health initiatives and communication.
• Contributes to process improvement and helps achieve departmental KPI goals through innovation and collaboration.
