Responsibilities include coordinating with EMR systems, generating performance reports, and ensuring data accuracy while adhering to healthcare policies and privacy requirements.

Formación

• High School Diploma or GED required.
• 2+ years working with an EMR system preferred.
• 2+ years experience in a medical office setting.
• Familiarity with data reporting and records management.

Responsabilidades

• Contact patients to meet outreach program goals via phone, email or mailings.
• If requested by Clinician, place lab orders and schedule appointments in EMR.
• Generate reports to identify patients needing additional labs or overdue labs.
• Collaborate with clinicians to determine required population reports and patient lists.
• Coordinate with care teams to optimize patient care and monitor service needs.
