Job Description
The Population Health Nurse provides high-quality, comprehensive, professional nursing care to individuals throughout the lifespan. The PHN offers patients both general and focused health education regarding preventive, chronic, and episodic health care issues.
Role and Responsibilities
- Outreach to patients to provide health care education during visits, via video, and by telephone.
- Follow up on emergency room visits and hospital discharges.
- Accurately document all aspects of patient care in the medical chart promptly.
- Complete medication reconciliation processes.
- Retrieve hospital discharge summaries from the appropriate portal and upload into patient records as needed.
- Address care gaps related to value-based metrics as assigned.
- Perform other duties delegated by the Population Health Department Director or Supervisor.
Qualifications
Required Skills and Education
- Registered nurse in good standing.
- Minimum of a Bachelor’s Degree.
- At least 1 year of successful healthcare experience.
- Ability to recognize signs of patient health distress and respond appropriately.
- Familiarity with medication names and ability to read prescriber’s directions.
- Comfort with technology; ability to listen and type concurrently into patient records accurately.
- Strong organizational skills and accuracy.
- Excellent communication and customer service skills.
Preferred Skills
- Current Connecticut RN license.
- Bachelor’s Degree in Nursing (BSN) preferred.
- Fluency in English and Spanish, with passing fluency exams.
- Over 1 year of ambulatory care nursing experience preferred; strong interest in chronic disease management and primary care encouraged.
- Commitment to underserved populations.
- Excellent written and oral communication skills.
- Experience with motivational interviewing techniques.
Required Licenses/Certifications
- Connecticut Registered Nurse License.
Physical Requirements and Work Environment
- Daily computer and telephone use.
- Alternating sitting and standing as workspace allows.
- Manual dexterity for administrative tasks; minimal physical activity.
- Remote work requires a clean, organized, distraction-free space, compliant with HIPAA and CHC policies.
