Job summary

Oak Street Health, part of CVS Health, is seeking a Population Health Manager in the Bronx, NY. You will coordinate and implement Population Health priorities across centers, working with providers, care teams, and operations to improve quality and reduce costs.

This fast-paced role reports to the Regional VP and the central Population Health team. You will lead analytics, program design, and implementation for transitions-in-care, medication adherence, and cost management, with focus on

Qualifications

• Minimum of 3 years in healthcare/population health.
• Strong communication skills.
• Ability to build trust with diverse stakeholders.
• Leadership potential to guide teams on priorities.
• Demonstrated history of creative problem-solving.
• Strong organization and attention to detail.
• Proficient in Excel and Google Apps.
• Scribing experience a plus, not required.
• US work authorization.

Responsibilities

• Lead strategy, execution, and refinement of Population Health programs in the region.
• Collaborate with regional leaders and center care teams to support the Oak Street care model.
• Develop scalable solutions for accurate clinical documentation and admission prevention.
• Partner with central teams on strategy and program design for documentation and cost efforts.
• Oversee analytics, insights, and project management to drive execution in the region.
• Own recruitment, retention, and engagement of medical scribes in the region.
• Align with key population health metrics: quality, utilization, and cost.
