Job summary

Apicha Community Health Center in Manhattan, NY seeks a Population Health Manager to join our team. You will drive value-based care initiatives, analyze clinical trends, and support program development aligned with our mission to improve community health for underserved populations.

Qualifications

Bachelor’s degree in Nursing, Health Care Administration, or related field.
Advanced degree preferred (MS/MPH/MA).
Experience in health care quality and performance improvement.
Strong data analysis and reporting skills.
Familiarity with UDS, HEDIS, NCQA PCMH accreditation standards.
Excellent verbal and written communication with attention to detail.

Responsibilities

Project Management: develop scopes and statements of work with the Director.
Monitor milestones, resolve issues, and coordinate status meetings with stakeholders.
Develop workplans and protocols to guide team activities and goals.
Coordinate data collection and reporting with value-based initiatives.
Analyze healthcare data to identify best practices and trends.
Validate data using EHR and population health systems and identify data discrepancies.
Support data integrity and quality assurance of reports (NCQA PCMH, HEDIS, UDS).
Lead quality improvement initiatives using RCA, PDSA, DMAIC.
Collaborate with clinicians to meet external reporting requirements.
Supervise the Quality Improvement Coordinator.
