Position Description:
The Population Health Manager plays a critical role in building, launching, and scaling our clinical programs and quality improvement initiatives by ensuring clinical programming improves the quality of care, health outcomes, and cost of care for the children and families that we serve.
Working closely with the Head of Clinical Strategy and Operations, this position seeks opportunities to utilize their population health expertise to identify scalable solutions across practices.By collaborating with various departments such as Technology, Operations as well as Practice Care Management and Quality teams, this position is responsible for developing best practice workflows, building market-facing materials, and training our population health teams to successfully execute programs.
This is a face-paced, cross-functional, highly visible role that ensures our innovative clinical model leads the industry in delivering comprehensive, coordinated care for children. The ideal candidate will bring excellent cross-functional communication, clinical operations expertise, and a passion for improving pediatric health outcomes in a value-based care environment.
Key Responsibilities:
Build clinical programs
- Support the design of population health programs to address preventive care, chronic condition management, behavioral health, developmental concerns, health-related social needs, and quality gap closure.
- Develop scalable workflows for new clinical programs in partnership with technology, operations, and local population health teams.
- Collaborate with technology vendors to enable programming – i.e., build telephony campaigns, write specification documents, and/or design customized dashboards for vendor engineering teams to build.
- Develop policies, procedures, and standard operating protocols to ensure consistency, quality, and compliance across practices.
- Create training materials and resources for local population health and provider teams to drive program adoption and consistency.
Launch clinical programs
- Provide training and ongoing support to local population health and provider teams to ensure consistent execution of clinical programs.
- Operationalize policies, procedures, and standard operating procedures across practices by embedding them into clinic workflows and job aids.
Scale clinical programs
- Develop implementation toolkits, playbooks, and templates to support efficient roll-out and replications of programs in new practices.
- Standardize key performance metrics and reporting processes to track progress and ensure accountability across practices.
- Supervise and mentor local population health and provider teams during program implementation to ensure fidelity of model, support professional development, and promote best practices across sites.
Qualifications:
- 5+ years of experience in population health management, care management, quality improvement, or clinical operations.
- Graduate degree preferred but not required (MBA, MPH, MHA, Nursing, or related field).
- Pediatric, Medicaid, and/or value-based care experience strongly preferred.
- High emotional intelligence with excellent interpersonal skills and ability to build trust and buy-in across interdisciplinary teams.
- Proactive communicator who puts individual projects into broader organizational objectives and can communicate their relevance to varying stakeholders.
- Strong organizational skills and ability to prioritize and follow through on tasks and projects with competing timeframes. Incredible level of detail orientation.
- Analytical and data savvy. Ability to design, monitor, and analyze KPIs and contextualize implications within the broader organizational strategy.
- Ability to develop digestible materials describing complex systems or topics for staff at varying levels of education and experience.
- Expert in Excel, PowerPoint, and project management.
- Ability to thrive in a fast-paced, dynamic environment and adapt to changing priorities.
- High level of integrity, humility, and professionalism.
