Job summary

Heritage Health is seeking a Population Health & Quality Analyst to drive patient-centered care through meaningful data insights and quality improvement initiatives. You will partner with clinical, operational, and leadership teams to analyze healthcare data, identify opportunities for improvement, and support strategies that enhance patient outcomes across our communities.

Qualifications

• Three years’ experience in data analysis and quality improvement in primary healthcare.
• Bachelor’s degree preferred.
• Equivalent combination of education and experience may qualify.

Responsibilities

• Analyze clinical quality data from EHRs and reports to inform population health priorities.
• Develop and maintain dashboards to monitor quality metrics.
• Prepare reports for leadership and payers to align with strategic goals.
• Collaborate with clinical and operational teams to implement improvement strategies.
• Lead population health action plans and track progress.
