Job summary

Care Alliance Health Center is seeking a Director of Quality & Population Health to lead performance improvement across the organization. You will translate data into actionable priorities to enhance patient care, safety, and equity while advancing value-based care initiatives.

The role requires strategic leadership, deep experience in population health, and a track record of driving care gap closure in diverse populations.

Qualifications

• Bachelor's degree in public health, healthcare administration, nursing, health sciences or related field.
• Minimum five years of progressive experience in healthcare quality, population health, risk management, patient safety, or related performance functions.
• Experience with payer quality measures, care gap management, and value-based programs.
• Experience with EHR data validation, clinical documentation, and quality reporting.
• Strong data analysis and reporting skills (Excel, dashboards).

Responsibilities

• Lead quality improvement, patient safety, and risk management strategy.
• Develop population health strategies to close care gaps across settings.
• Monitor quality and population health requirements across payer/ACO arrangements.
• Oversee FTCA risk management requirements and reporting.
• Analyze performance data to identify trends, risks, and improvement opportunities.
• Identify patients needing preventive screening and care coordination.
• Develop and maintain policies related to quality and risk functions.
• Provide leadership to QA & Risk Management team.
