Northern Counties Health Care, Inc. seeks a Population Health Data Analyst to develop and maintain dashboards and reports supporting population health, clinical quality improvement, and organizational decision-making.

You will work with clinical, operational, and quality leaders to turn data into reliable information, focusing on data governance, automation, and end-user support in a health-centered environment.

Qualifications

• Bachelor's degree in a health analytics or related field preferred.
• 2+ years of data analytics or healthcare analytics experience preferred.
• Experience building dashboards in Power BI and reporting automation helpful.
• Knowledge of healthcare data, eCQMs, population health or FQHC reporting is a plus.
• Ability to extract, transform, validate, reconcile, and analyze data.

Responsabilités

• Clarify reporting needs with leadership and prioritize work.
• Develop, maintain, and improve dashboards, scorecards, data models, and reports.
• Extract, transform, reconcile, and validate clinical and operational data.
• Support eCQM reporting and data validation with stakeholders.
• Build Community Health Team reports for referrals, encounters, and outcomes.
• Automate recurring reports and document processes for continuity.
• Analyze trends, explain findings, and identify opportunities for improvement.
• Align reporting with EHR data and governance, and collaborate with teams.
