Northern Counties Health Care Inc. is seeking a Population Health Data Analyst in St. Johnsbury, VT to design and maintain reports, dashboards, and data tools supporting population health, quality improvement, and care coordination.

The role collaborates with clinical, operational, and quality leaders to turn data into actionable information, focusing on data analysis, reporting, validation, automation, and user support within governance standards.

Qualifications

• Bachelor’s degree in health informatics, data analytics, information systems, public health, healthcare administration, business analytics, or a related field preferred
• Two or more years of experience in data analytics, business intelligence, healthcare analytics, clinical informatics, or a related field preferred
• Experience developing dashboards and reports using Microsoft Power BI or a comparable platform strongly preferred; experience with advanced Excel, relational data, SQL, Power Query, DAX, data modeling, or report automation desirable
• Experience with healthcare data, EHR data, or clinical reporting preferred; athenaOne experience a plus
• Ability to extract, transform, validate, reconcile, and analyze data and communicate findings clearly to technical and nontechnical audiences
• Strong analytical, organizational, and problem-solving skills; ability to manage priorities, meet deadlines, and communicate barriers promptly
• Ability to collaborate across clinical, operational, technical, and administrative teams and provide training and support
• Attention to detail, initiative, adaptability, continued learning, and sound judgment in handling confidential information

Responsibilities

• Clarify reporting needs with the Director of Clinical Informatics and Population Health Manager, prioritize work, establish timelines, and support quality and grant reporting requirements
• Develop, maintain, and improve patient panels, dashboards, scorecards, data models, and standardized reports using Power BI and approved tools
• Extract, transform, reconcile, and validate clinical and operational data; investigate discrepancies and coordinate corrections
• Support eCQM reporting through data validation, monitoring, and troubleshooting with clinical leaders
• Build and maintain Community Health Team reports and tracking tools for referrals, encounters, interventions, workload, and outcomes
• Automate recurring reports and improve reporting efficiency; maintain documentation for staff support
• Analyze trends, explain findings and data limitations; help managers use information for quality and operational improvement
• Align reporting with CEHRT data; refer EHR configuration needs to Director of Clinical Informatics
• Collaborate with practice managers, providers, and quality staff to make reporting tools practical
• Provide dashboard training and end-user education; explain report definitions and limitations
• Maintain report specifications, data definitions, validation records, and user resources in line with data governance
• Participate in cross-training and knowledge transfer; coordinate with external partners as assigned
• Protect patient, employee, and organizational information per HIPAA and policies
• Perform other related duties as assigned
