You will analyze performance data, support improvement initiatives, and mentor the Population Health team while collaborating with clinic operations, analytics, health IT, and executive leadership.

Qualifications

• Bachelor’s degree in healthcare or related field or equivalent.
• Current driver’s license and Hawai‘i auto insurance.
• Two years of experience in health care, population health, or equivalent healthcare experience.

Responsibilities

• Provide population health and quality metrics support for Hawai‘i Health Partners Primary Care Physician members.
• Analyze physician practice performance data to support decision‑making and program implementation.
• Mentor Population Health team members with more complex assignments.
• Collaborate with clinic operations, analytics, health IT, and senior leadership to improve outcomes.
