Job summary

Sea Mar Community Health Centers seeks a Health Informatics and Metrics Project Coordinator to advance organizational quality initiatives around the EHR, patient outcomes, and operational processes. You will work with the Quality Improvement Medical Director, IT, and clinical staff to support Meaningful Use, HEDIS, PCMH, and UDS reporting.

Responsibilities include auditing CQM data, developing dashboards in Excel/SharePoint, and presenting findings to leadership.

Qualifications

• Knowledge of HEDIS, Meaningful Use, UDS, and PCMH measures.
• Proficient in data extraction, interpretation, and presentation.

Responsibilities

• Collaborate with multidisciplinary teams to advance quality initiatives.
• Audit clinical quality measure data and report findings to management.
• Develop graphs and dashboards to track quality measures and population health.
• Coordinate with IT and clinic staff to support quality projects and initiatives.
• Prepare meeting minutes and maintain SharePoint pages.
