Job summary

The role provides health coaching and collaborates with care teams to coordinate services, collects data in the EHR for tracking and NCQA initiatives, and supports wellness initiatives for patients.

Qualifications

• High School Diploma or equivalent; Medical Assisting or Nursing degree preferred.
• Minimum of one year experience of practice within a Valley Health Systems health center.
• Working knowledge of Medicaid/Medicare, adult learning and behavior change principles, chronic illnesses, co-morbidities and disease management and community resources.
• Excellent verbal and written communication, telephonic interviewing, computer, and organizational skills.

Responsibilities

• Provide health-coaching and consultation services to patients within licensure scope of practice requirements.
• Collaborate with patients to establish goals and provide ongoing support for healthy lifestyle changes.
• Collaborate with primary care staff and provider to obtain health information for the member's plan of care.
• Review data from various sources to identify health issues and plan interventions.
• Assist in care coordination including test and referral results and communication with community entities and health plans.
• Proactively address patient needs through population health management.
• Create and manage patient registries for chronic disease and preventive care.
• Document all activities per Valley Health Policy Manual.
• Assist operations in collecting clinical data for NCQA accreditation and meaningful use.
• Other duties as assigned.
