Our Client is seeking an Ambulatory Clinical Pharmacy Specialist in Dover, DE to join a permanent team. The role focuses on comprehensive ambulatory care under collaborative practice agreements, collaborating with providers to optimize outcomes and reduce costs.

Candidates must hold a BPS Pharmacy specialty certification and have at least one year of clinical experience. Salary ranges from six figures with potential bonus considerations and relocation options may apply.

Qualifications

• Bachelor degree in Pharmacy is required.
• Licensed Pharmacist with BPS certification in a specialty such as Pharmacotherapy, Ambulatory Care, Cardiology, or Geriatrics.
• At least one year of clinical pharmacy experience, including residency experience.

Responsibilities

• Provide ambulatory clinical pharmacy services under CPA as credentialed by MEC.
• Conduct patient visits for medication management, counseling, and monitoring.
• Collaborate with providers to implement evidence-based medication interventions.
• Address referrals ensuring closed-loop communication with clinicians.
• Support access and affordability via prior authorizations and financial assistance.
• Oversee patients on specialty medications for safe initiation and monitoring.
• Provide drug information and education to patients and healthcare teams.
• Precept students, interns, or residents as assigned.
• Develop or improve systems to enhance patient care.
• Participate in research/publications related to ambulatory and specialty pharmacy.
• Engage in policy development, quality initiatives, and committee work.
• Document interventions in the EHR and report outcomes per standards.
