Daily duties include outreach by phone and text, scheduling appointments, and supporting clients through barriers while maintaining HIPAA-compliant documentation in the EMR.

Formación

• High School Diploma or GED required.
• Successful completion of a Community Health Worker certification training program.
• Accredited by the Maryland Department of Health at time of hire or within five months after probation.
• One year experience in a healthcare setting.
• Bi-lingual abilities are a plus.
• Some college is preferred.

Responsabilidades

• Provides education, coaching, and support to clients to achieve health goals and close care gaps.
• Outreach via phone and text messaging to schedule appointments and address barriers.
• Collaborates with RN Care Coordinators, practitioners, and care teams.
• Schedules appointments, inputs registration information, and ensures continuity of care.
• Documents care coordination and phone notes in EMR and uses HIPAA-compliant platforms.
