Responsibilities

• Coordinate population health programs and interventions with care teams.
• Manage a panel of patients, engage patients into care and schedule recommended appointments.
• Track patient lists for needed care and generate reports as requested.
• Assist with pre-visit information, reminders, and addressing barriers to attendance.
• Support team communications, referrals, and coordination with community resources.
