Job summary

People Incorporated Mental Health Services seeks a Population Health Care Coordinator to advance whole-person health within outpatient teams. The role integrates care for clients with co-morbid behavioral, physical, and mental health conditions, focusing on long-term engagement and health promotion.

The PHCC collaborates with interdisciplinary teams to identify health needs, implement interventions, and support clients toward individualized goals, with duties spanning client engagement,

Qualifications

• Master’s degree orBachelor’s degree with practicum/internship and direct client interaction required.
• 450–2000 hours of relevant experience depending on educational path is required.
• Experience with behavioral health, co-morbidity, or population health practices preferred.

Responsibilities

• Coordinate care for clients with co-morbid conditions, ensuring continuity across services.
• Engage clients in skills development, motivation, and problem-solving to improve health outcomes.
• Collect, analyze, and report population health data to inform program planning and quality improvement.
• Collaborate with interdisciplinary teams to resolve client issues and enhance engagement.
• Document all client interactions and population health activities in the EHR.

Education

Master’s degree in a behavioral science or related field

Bachelor’s degree in a behavioral science or related field with practicum or internship

30 college credits in behavioral sciences

1800–2000 hours of experience with mental illness or related conditions

4,000 hours of experience with mental illness/traumatic brain injury with supervision
