Job summary

Qualifications

• BSN or MSW required.
• 3 years of hospital case management experience preferred.
• Must obtain ACM/CCM/ANCC certification within 2 years of hire.

Responsibilities

• Monitor daily census and assign patients for assessment.
• Coordinate discharge planning and care transitions.
• Communicate with care teams about assessment findings.
• Maintain documentation per guidelines and report requests.
• Develop relationships with internal and external stakeholders.
