Responsibilities

• Monitor daily census and assignment to ensure all patients are assessed.
• Case management includes coordination, transition, and discharge planning across settings.
• Maintain knowledge of benefits, discharge process, and CMS regulations.
• Provide education and guidance to providers, patients, and families.
• Collaborate with Utilization Management to improve payer negotiations and denial prevention.
• Document assessments and discharge actions per department guidelines and generate reports.
• Participate in QAPI activities as requested.
• Site-specific duties may include weekend/holiday coverage.
• Build positive relationships with internal and external customers.
