Job summary

Jefferson is seeking a Population Health Care Coordinator LPN to collaborate with the ambulatory care team. You will develop and implement individualized care plans, coordinate across home, physician offices, hospital, and other settings, and support patient education and safe transitions of care.

The role emphasizes patient-centered care, effective communication, and adherence to clinical protocols to improve health outcomes and reduce costs for identified populations.

Qualifications

• Specialized Diploma LPN with 7 years of nursing experience.
• Knowledge of patient- and family-centered practice and culturally sensitive care.
• Ability to convey respect and collaborate with patients and families in care planning.
• Strong communication and organizational skills for education and care coordination.

Responsibilities

• Collaborate with the ambulatory care team to coordinate care across home, physician office, hospital and other settings.
• Identify learning needs with patients and families and provide education.
• Develop and manage care coordination for medium/high risk patients.
• Ensure timely transitions of care and follow-up after hospital discharge.
• Maintain documentation and use health IT to support care coordination.
