Kids for the Future seeks a Population Health Assistant to support the Director of Quality in managing payer programs and quality initiatives. You will work with Athena and Azara to generate reports, analyze data, and close care gaps across clinics. Strong Excel/Word skills and EHR experience are essential.

The role emphasizes collaboration with sites and participation in quality improvement activities, with full-time, non-exempt status and compensation based on experience and credentials.

Qualifications

• Licensed or credentialed medical assistant.
• 3–5 years of outpatient, family medicine experience.
• Experience with electronic health records (EHR).
• Proficient use of email and online reporting systems.
• Highly organized with ability to keep accurate notes and records.

Responsabilités

• Utilize Athena and Azara to run reports and analyze quality data.
• Work with sites to close patient care gaps by generating reports and data.
• Assist with managing third-party payer programs and communicating with contacts.
• Assist with managing Accountable Care Organization requirements (Aledade) and liaising with specialists.
• Maintain proper medical documentation and data integrity.
