Job summary

Sunnyvale Gardens Post Acute is seeking a dedicated Registered Nurse (RN) for the PM shift in Sunnyvale, CA. This full-time role offers a competitive pay rate and a comprehensive benefits package in a 140-bed skilled nursing and rehabilitation facility.

You will administer medications, perform assessments, and provide direction to CNAs/LVNs while collaborating with physicians and families to ensure high-quality resident care.

Qualifications

• Current, unrestricted California RN license.
• Current BLS/CPR certification.
• Previous skilled nursing, post-acute, rehabilitation, or long-term care experience preferred.
• Strong clinical judgment, communication, and organizational skills.

Responsibilities

• Provide compassionate, high-quality clinical care to short-term rehabilitation and long-term care residents.
• Administer medications, treatments, and IV therapies safely and accurately.
• Perform resident assessments and monitor for changes in condition.
• Accurately document clinical findings, interventions, and resident status.
• Provide direction and support to CNAs and LVNs.
• Communicate effectively with physicians, residents, families, and the interdisciplinary team.
• Respond appropriately to clinical concerns and emergencies.
• Promote a safe, respectful, and resident-centered environment.
