Job summary

Sunnyvale Gardens Post Acute is a 140-bed skilled nursing and rehabilitation facility in Sunnyvale, CA, seeking dedicated Registered Nurses for PM and NOC shifts. The role is full-time with a 4 days on, 2 days off rotating schedule, offering comprehensive benefits and competitive pay.

Responsibilities include administering medications, performing resident assessments, documenting care, and guiding CNAs and LVNs. California RN license and BLS/CPR are required; new grads are welcome to apply.

Qualifications

• Current, unrestricted California RN license.
• Current BLS/CPR certification.
• New graduates are welcome to apply.
• Strong clinical judgment, communication, and organizational skills.

Responsibilities

• Provide compassionate, high-quality clinical care to short-term rehabilitation and long-term care residents.
• Administer medications, treatments, and IV therapies safely and accurately.
• Perform resident assessments and monitor for changes in condition.
• Accurately document clinical findings, interventions, and resident status.
• Provide direction and support to CNAs and LVNs.
• Communicate effectively with physicians, residents, families, and the interdisciplinary team.
• Respond appropriately to clinical concerns and emergencies.
• Promote a safe, respectful, and resident-centered environment.
