Job summary

Evergreen is seeking a compassionate Registered Nurse (RN) to join our 2nd shift team in a short-term rehab setting. The role offers competitive hourly pay and a comprehensive benefits package, with a 2.30pm–10.45pm PM shift and 30–40 hours per week.

As part of a supportive, collaborative team, you will assess residents, administer medications, and develop care plans while maintaining thorough documentation and pursuing ongoing education.

Qualifications

• Active Wisconsin RN license.

Responsibilities

• Provide compassionate, high-quality nursing care in a short-term rehabilitation environment.
• Assess resident needs and develop individualized care plans.
• Administer medications and treatments per physician orders.
• Monitor resident conditions and evaluate care outcomes.
• Collaborate with physicians, therapists, nursing staff and interdisciplinary team.
• Build trusting relationships with residents and families through education and communication.
• Maintain accurate and timely clinical documentation.
• Participate in ongoing education and professional development.
