Comprehensive Rehab Consultants (CRC) is hiring a PM&R clinician to provide specialized Physiatry services in SNF/post-acute settings in Nacogdoches and Lufkin, Texas. The role is a 32-hour W-2 position with autonomy over daily workflow.

You will collaborate with interdisciplinary teams, support patient independence, and help manage conditions common in geriatric post-acute care. Competitive salary, benefits, and ongoing education are included.

Qualifications

• Nurse Practitioner/APRN/PA license required.
• Texas license required.
• Ability to work in the U.S. is required.
• Experience in post-acute care space preferred.

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with clinical leadership and collaboration with the care team.
• Collaborate with PCP teams to manage conditions common in SNFs and post-acute settings.
• Coordinate with PT/OT/SLP teams to improve patient function and quality of life.
• Follow therapy patients to track progress and address barriers to therapy.
• Participate in multidisciplinary meetings and support culture-building initiatives.
