Comprehensive Rehab Consultants (CRC) seeks a Part-time PM&R Clinician (NP/PA) in Hobart, IN to provide specialized PM&R/Physiatry services at a designated SNF or post-acute rehab center. You will lead interprofessional teams and help optimize rehabilitative outcomes, promoting patient independence.

Requirements include NP/PA license, Indiana licensure, and US work authorization. Experience in post-acute care is preferred; hours around 9 AM–4 PM with a 8-hour shift and weekday availability.

Qualifications

• Nurse Practitioner/APRN/PA license and Indiana licensure required.
• US work authorization required.
• Post-acute/SNF experience preferred.
• Demonstrated patient-centered care and collaboration with care teams.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in the facility with strong clinical leadership.
• Collaborate with PCP teams for SNF/post-acute conditions.
• Work with PT/OT/SLP to improve patient function and quality of life.
• Follow patients to track progress in therapy.
• Address barriers to therapy and optimize recovery.
