Job summary

Comprehensive Rehab Consultants (CRC) in Hobart, IN, seeks a PM&R/Physiatry clinician (NP/PA/APR). The part-time W2 role serves a designated SNF, collaborating with the post-acute therapy team to optimize rehab outcomes and patient independence.

You will work with PCP, PT, OT, and SLP teams, lead clinical care, attend multidisciplinary meetings, and access CRC training and certification opportunities. Indiana license and US work authorization are required.

Qualifications

• NP/APRN/PA license required in Indiana.
• Ability to work in the U.S.
• Post-acute/SNF experience preferred.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with clinical leadership.
• Collaborate with PCP teams to manage SNF conditions.
• Work with PT/OT/SLP to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
