Comprehensive Rehab Consultants in St. George, UT, is seeking an NP/PA PM&R clinician to provide specialized post-acute rehab services exclusively at SNFs. You will collaborate with inter-disciplinary teams to optimize patient outcomes and independence.

Qualifications

• Nurse Practitioner/APRN/PA license required; Utah license preferred.
• Experience in post-acute/SNF settings preferred.
• Ability to operate autonomously with strong communication.
• Empathy and patient-centered care mindset.

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility through clinical leadership and collaboration.
• Collaborate with PCP teams to manage SNF conditions (CVA, neurological, post‑operative).
• Work with PT/OT/SLP to improve function and quality of life.
• Follow therapy patients to track progress and outcomes.
• Assess and address barriers to therapy (pain, spasticity, neurogenic issues).
• Participate in multidisciplinary meetings and contribute to culture and education.
