Comprehensive Rehab Consultants in Buchanan, GA, Carrollton, GA is seeking a Nurse Practitioner/PA to provide PM&R/Physiatry services at a designated SNF or post-acute rehab center in a full-time W-2 role.

The ideal candidate will lead with clinical excellence, collaborate with the care team, and support patient independence while enjoying a comprehensive benefits package and opportunities for growth.

Qualifications

• Nurse Practitioner/APRN/ PA credential required.
• Georgia license required.
• Ability to work in the U.S.
• Post-acute/post-acute space experience preferred.
• 40 hours per week.

Responsibilities

• Provide specialized Physical Medicine and Rehabilitation (PM&R)/Physiatry services as a rehab consultant.
• Represent CRC in your facility through strong clinical leadership and collaboration with the interdisciplinary care team.
• Collaborate with PCP teams to help manage conditions common in SNFs such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries.
• Work alongside PT, OT, and SLP teams to improve patient function and quality of life.
• Follow therapy patients to track their progress in therapy.
• Assess and collaborate on management of barriers to therapy including pain, spasticity, neurogenic bowel/bladder, and more.
• Attend multidisciplinary meetings and bridge the gap between therapy and internal medicine; engage in culture-building and ongoing education.
