Job summary

Comprehensive Rehab Consultants seeks a Nurse Practitioner/PA to provide PM&R/Physiatry services at designated SNFs in Rockville, Cromwell, and Middletown, CT. This is a full-time W-2 role with autonomy, collaboration with therapy and PCP teams, and a focus on geriatric post-acute care.

Qualifications

• Connecticut License and ability to work in the U.S.
• Experience in post-acute space preferred.
• Nurse Practitioner/APRN/ PA certification required; able to work 40 hours/week.

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility through strong clinical leadership and collaboration with the interdisciplinary care team.
• Collaborate with PCP teams to help manage conditions common in SNFs (e.g., CVA, progressive neurological conditions, postoperative orthopedic surgeries).
• Work with PT, OT, and SLP teams to improve patient function and quality of life.
• Follow therapy patients to track progress in therapy.
• Assess and manage barriers to therapy (pain, spasticity, neurogenic bowel/bladder, etc.).
• Be an integral part of the SNF team by attending multidisciplinary meetings and bridging therapy with internal medicine.
• Engage in culture-building opportunities and ongoing education (Didactic lectures, regional meetings, annual dinners).
