Job summary
Endeavor Health is seeking an Advanced Practice Provider in the Neurology Department for outpatient and hospital-based care in Skokie, IL. This full-time role requires establishing patient histories, performing exams, ordering tests, and developing treatment plans under physician supervision.
Eligible candidates will hold an APN/PA license in Illinois with DEA and controlled substances certification, a master’s degree in a PMR-related field, and BLS/CPR certification; prior PMR/pain management
Qualifications
- APN or PA licensure in IL required; Federal DEA and IL Controlled Substance License required.
- Master's level education in specialty area.
Responsibilities
- Provide care to patients in Neurology Department under physician supervision.
- Take detailed history and perform physical examinations; order and interpret imaging, labs, and diagnostics.
- Prescribe pharmacologic and non-pharmacologic therapies; develop and adjust treatment plans.
- Maintain records and progress notes; ensure continuity of care in clinic and hospital settings.
