Endeavor Health is seeking an Advanced Practice Provider in Physical Medicine & Rehabilitation in Skokie, IL. This full-time role involves working in a Neurology/PMR setting under collaborating physicians, with responsibilities spanning patient history, exams, orders, documentation, and interdisciplinary collaboration.

The position requires IL APN/PA licensure, DEA license, and a master's degree in PMR or related field, plus BLS/CPR certification.

Qualifications

• Must hold APN or PA license in Illinois with DEA and Illinois Controlled Substance license.
• Master's level degree in PMR or related specialty.
• BLS/CPR certification required.
• Prior APN/PA experience preferred.
• 1 year PMR or pain management experience preferred.

Responsibilities

• Provide care under the direction of collaborating physician(s) in Neurology/PMR.
• Obtain detailed history and perform physical examinations; order imaging and labs and interpret results.
• Instruct patients on prescribed regimens and pre/postoperative instructions.
• Maintain records and write progress notes; follow patients through clinic and hospital settings.
• Collaborate with physicians and health care team to achieve optimal outcomes; prescribe therapies as appropriate.
• Demonstrate evidence-based practice and participate in standards of practice development.
• Assess learning needs of patients and families and provide education to promote health.
