Job summary

Endeavor Health in Skokie, IL is seeking an Advanced Practice Provider in Physical Medicine & Rehabilitation to join the Neurology Department. This full-time outpatient position emphasizes comprehensive patient assessments and coordinated care within an interdisciplinary team.

You will perform histories and exams, order imaging and labs, develop treatment plans, and educate patients on therapies, always following evidence-based protocols and organization policies.

Qualifications

APN/PA licensure in IL required
DEA and Illinois Controlled Substance licenses required
Master's degree in PMR specialty or related field
BLS/CPR certification required

Responsibilities

Provide care to patients under collaborating physician(s)
Obtain detailed history and perform physical examinations; order imaging, labs, and diagnostic procedures
Interpret results and write progress notes; manage treatment course in clinic and hospital
Prescribe pharmacologic and non-pharmacologic therapies per protocol
Collaborate with physicians and health care team to achieve optimal outcomes and promote wellness
