Job summary

Avina Healthcare in Wisconsin is seeking a Certified Nursing Assistant (CNA) to provide routine daily nursing care to residents per care plans and supervisor directions.

You will assist with activities of daily living, mobility, grooming, dining, and monitoring changes in condition, reporting to the nurse promptly.

The role emphasizes teamwork, infection control, dignity, and compassionate support for residents and families.

Qualifications

• Must have current and active nursing aide certification in Wisconsin.

Responsibilities

• Resident centered care oriented including assistance with activities of daily living such as:
• Positioning, Moving, and Restorative Care.
• Mobility, Toileting, and Bathing.
• Grooming: Oral, Shaving and Nail Care.
• Dining Assistance.
• Dressing and Undressing.
• Infection control and prevention.
• Nutrition and Elimination.
• Comfort care and End of Life Care.
• Answer call lights timely and promptly.
• Report all changes in the resident's condition to your assigned nurse as soon as possible.
• Report all accidents and incidents you observe on the shift when they occur.
• Record all entries on flow sheets, notes, charts, etc., in an informative and descriptive manner.
• Be a team player working in coordination with all disciplines in the facility.
