Responsibilities

• Provide routine daily nursing care and services per resident assessment and care plan.
• Assist residents with activities of daily living (ADLs) including positioning, mobility, toileting, bathing, grooming, dining, dressing.
• Ensure infection control and prevention in daily tasks.
• Report changes in resident condition to assigned nurse promptly.
• Document entries on flow sheets and charts clearly and accurately.
• Be a team player and coordinate with all facility disciplines.
• Respond to call lights promptly and assist as needed.
• Support end-of-life care and comfort measures as required.
• Maintain clean, safe, and respectful resident environment.
• Follow facility policies and care plans at all times.

Resident centered care oriented including assistance with activities of daily living such as:

• Positioning, Moving, and Restorative Care.
• Mobility, Toileting, and Bathing.
• Grooming: Oral, Shaving and Nail Care.
• Dining Assistance.
• Dressing and Undressing.
• Infection control and prevention.
• Nutrition and Elimination.
• Comfort care and End of Life Care.
• Answer call lights timely and promptly.
• Report all changes in the resident's condition to your assigned nurse as soon as possible.
• Report all accidents and incidents you observe on the shift when they occur.
• Record all entries on flow sheets, notes, charts, etc., in an informative and descriptive manner.
• Be a team player working in coordination with all disciplines in the facility.
