Lutheran Social Services of Wisconsin & Upper Michigan is seeking a Care Coordinator for the CLTS Waiver Program in Waukesha County. You will help families access services and develop individualized plans, traveling to communities as needed.

This full-time role offers remote flexibility, strong team collaboration, and opportunities for professional growth within a federally funded program serving children with disabilities.

Qualifications

• Bachelor's degree in a human services field (e.g., Social Work, Psychology, etc.).
• 1 year minimum experience with children with disabilities.
• Spanish bilingual preferred but not required.

Responsibilities

• Assess children's functional abilities using approved tools.
• Develop and implement individualized service plans with families and providers.
• Facilitate team meetings and coordinate services based on family-centered goals.
• Maintain accurate documentation and meet regulatory timelines.

Lutheran Social Services of Wisconsin & Upper Michigan is seeking a compassionate and organized Care Coordinator to join our CLTS Waiver Program team in Waukesha County! The Children's Long-Term Support (CLTS) Waiver Program is a federally funded Medicaid initiative that helps children with developmental, physical, or severe emotional disabilities receive services that allow them to thrive in home and community settings. As a Care Coordinator, you'll be the primary point of contact for families, helping them access and navigate essential services. Please note: This is a continuous posting in anticipation of future growth opportunities in the Waukesha County area.
