Job summary

Guidehealth, a data-powered, performance-driven healthcare company, is seeking a Practice Success Manager to own relationships with physician practices across a defined portfolio. You will lead engagement from introduction through implementation, coordinating with CDI, CM, and Quality leads to ensure value-based care delivery.

Based in or willing to relocate to the Atlanta, GA area, with travel up to 50% within Georgia for in-person engagement.

Qualifications

• Minimum 3 years in a practice-facing, account management, or business development role within healthcare, value-based care, or a related field.
• Experience working within an ACO, MSSP, or value-based care model (delegated services, managed care, or population health environments)
• Demonstrated ability to build trust-based relationships and secure buy-in from physicians and practice leadership
• Strong written and verbal communication skills, with the ability to translate performance data into a clear narrative for practice audiences

Responsibilities

• Identify and evaluate individual practices within assigned networks for fit with Guidehealth's value-based care services.
• Conduct outreach and initial meetings with individual practice leadership to introduce Guidehealth's services and value proposition
• Build the case for engagement and address practice-level objections to move into implementation
• Serve as the day-to-day contact for an assigned portfolio of practices with regular updates
• Document all practice meetings and key interactions in HubSpot CRM
• Schedule and coordinate practice-facing meetings, including performance reviews and escalations
• Collaborate with CDI, CM, and Quality leads to address practice needs and service delivery barriers
