Job Summary

Baptist AgeWell Center for Health is a senior-focused service that utilizes multiple modalities to provide exemplary care for our patients in Northeast Florida. Located downtown in the One Call Building on the Baptist Medical Center of Jacksonville campus, we're seeking a passionate Physician dedicated to senior patients. We offer our physicians the opportunity to have a meaningful impact on patients' lives without losing their way in the business of healthcare.

What You Can Expect

• Lead your care team, collaborating with experienced Advanced Practice Providers
• Support from an extensive in-practice team including behavioral health, social work, pharmacy, nutrition, care coordination, and rehab resources
• Locally governed, physician-led, professionally managed, and community-focused program ensuring autonomy and a voice in your practice
• Epic EHR
• M to F 8 am to 5 pm
• On call 2 to 3 weeks per year, Monday through Sunday

What You Will Need

• MD or DO
• Active/unrestricted medical license
• Board certified/eligible
• Family Medicine or Internal Medicine training with a passion for senior care and population health

Education

Required: Doctorate of Medicine

Experience

• Over 5 years of Family Practice experience preferred
• Experience working with geriatric populations preferred

License and Certification

• Basic Life Support (BLS) preferred
• Licensed Physician required
• DEA License required
