Job summary

Northeast Healthcare Recruitment, Inc. (NEHCR) is seeking an experienced Physician Assistant to join our clinical team in skilled nursing facilities. You will manage your own patient panel, provide primary and episodic care, and build lasting relationships with residents and families.

Qualifications

Responsibilities

• See roughly 16-20 patients a day, including wellness visits and acute issues.
• Manage chronic conditions, review meds, and coordinate with physicians and facility staff.
• Build long-term relationships with residents and families; participate in care conferences.

About the Role

We're looking for an experienced, autonomous Physician Assistant to join our clinical team, providing primary and episodic care to residents in skilled nursing facilities. This is an independent role built around continuity - you'll manage your own panel and build long-term relationships with patients and their families.

Your Day-to-Day

You'll see roughly 16-20 patients a day - a mix of long-term chronic care and acute issues. That means annual wellness visits and H&Ps, managing chronic and co-morbid conditions, responding to acute changes, reviewing meds and orders, and coordinating with collaborating physicians, specialists, and facility staff to keep care seamless. You'll also stay in touch with families and join care conferences where it makes sense.
