The position emphasizes comprehensive assessments, treatment planning, and education for patients and families, in a patient-centered, collaborative environment.

Qualifications

• Eligible to work in the United States.
• Active Illinois PA license preferred.
• Current Controlled Substance License preferred (required upon start date).
• Current DEA License preferred (required upon start date).
• NCCPA Certificate preferred.
• Interest in Physiatry/Physical Medicine and Rehabilitation preferred.
• Orthopedic Experience (nice to have).
• Physiatry Experience (nice to have).

Responsibilities

• Collaborate with therapists, nurses, and other professionals to develop and implement comprehensive treatment plans.
• Conduct thorough patient assessments, histories, physical exams, and diagnostics to set rehab goals.
• Monitor progress and adjust treatment plans to optimize function and independence.
• Participate in multidisciplinary meetings, case conferences, and educational activities.
• Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries in EMR.
• Provide education and counseling to patients and families about rehab processes and post-discharge care.
