Job summary

Chestnut Health is seeking a compassionate Physician Assistant to join our physiatry team. You will collaborate with therapists, nurses and other professionals to create and implement comprehensive rehabilitation plans tailored to diverse patient needs.

You will perform patient assessments, monitor progress, and adjust plans to maximize function and independence. Emphasis on patient education and multidisciplinary collaboration in a patient-centered setting.

Qualifications

• Eligible to work in the United States.
• Active Physician Assistant license (Illinois preferred).
• Current DEA license preferred (required upon start date).
• Current Controlled Substance license preferred (required upon start date).
• NCCPA certification preferred.

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
• Conduct thorough patient assessments, including medical histories and physical examinations, to determine rehabilitation goals and interventions.
• Monitor patient progress and adjust treatment plans to optimize functional outcomes and independence.
• Participate in multidisciplinary team meetings, case conferences, and educational activities.
• Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries in EMR.
• Provide education and counseling to patients and families regarding rehabilitation processes and post-discharge care.
