The role offers collaboration with therapists, nurses, and physicians to create comprehensive treatment plans, conduct detailed assessments, and monitor progress for optimal functional outcomes.

Qualifications

• Active license to practice in the state of employment (Illinois).
• Current Controlled Substance License required on start date.
• Current DEA License required on start date.
• NCCPA certification is required.
• Desire to work in Physiatry/PM&R.

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine rehabilitation goals and interventions.
• Monitor patient progress and adjust treatment plans to optimize functional outcomes and independence.
• Participate in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
• Maintain accurate documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
• Provide education and counseling to patients and families regarding the rehabilitation process, outcomes, and post-discharge care.
