Job summary

Qualifications

• Active PA license in the state of employment of this role.
• Current DEA license is required upon start date.
• NCCPA certification is required.

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive rehabilitation treatment plans.
• Conduct thorough patient assessments to determine rehabilitation goals and interventions.
• Monitor patient progress and adjust treatment plans to optimize functional outcomes and independence.
• Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries in electronic medical records.
• Educate patients and families regarding the rehabilitation process and post-discharge care.
