Job summary

Chestnut Health Systems is seeking a Physician Assistant to join our Physiatry team. You will work in a collaborative, patient-centered environment to support rehabilitation goals and independence.

You will assess, document, and adjust treatment plans for diverse rehab needs while educating patients and families about the process and post-discharge care. New graduates are encouraged to apply.

Qualifications

• Eligible to work in the United States.
• Active Physician Assistant license in Illinois preferred.
• Current Controlled Substance License preferred (required upon start date).
• Current DEA License preferred (required upon start date).
• NCCPA Certificate preferred.

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement treatment plans.
• Conduct patient assessments, including medical histories, examinations, and diagnostic evaluations.
• Monitor progress and adjust plans to optimize functional outcomes and independence.
• Participate in multidisciplinary meetings and educational activities to enhance clinical skills.
• Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries in EMR.
• Provide education and counseling to patients and families regarding rehabilitation and post-discharge care.
