Qualifications

• Active license to practice in the state of employed
• Current Controlled Substance License required on start date
• Current DEA License required on start date
• NCCPA Certification
• Interest in Physiatry/PM&R
• Orthopedic experience a plus
• Physiatry experience a plus
• SNF experience a plus

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with rehabilitation needs.
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine rehabilitation goals and interventions.
• Monitor patient progress and adjust treatment plans to optimize functional outcomes and independence.
• Participate in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
• Maintain accurate and timely documentation of evaluations, treatment plans, progress notes, and discharge summaries in EMR systems.
• Provide education and counseling to patients and families regarding rehabilitation process, outcomes, and post-discharge care.
