LeoForce is seeking a Physician Assistant or Nurse Practitioner to provide comprehensive primary and ambulatory care in Hartford, CT, with NHSC loan repayment eligibility and substantial benefits. responsibilities include patient histories, assessments, and collaborative care with physicians and nurses.

You will develop evidence-based treatment plans, document thoroughly in the EHR, and contribute to quality measures while serving a diverse patient population in a nonprofit community healthcare

Qualifications

• Active Connecticut APRN or Physician Assistant license.
• Graduate of an accredited NP or PA program.
• DEA and CT controlled-substance registration, or eligible to obtain both.
• Provider practice experience preferred.
