Advanced Practitioners – Primary Care

Job Details

• Join a primary care team in a 100% outpatient setting focused on Adult Medicine (18+)
• Open to Nurse Practitioners (Family, Adult/Geri) and Physician Assistants
• Full‑time position with a Tuesday through Friday schedule – weekdays only with flexible work schedule options available
• No call required
• Build your own patient panel in a patient‑centered environment with ample support for seamless integration
• Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients
• Develop plans for further evaluation of health needs in consultation with physicians and care team members, including referrals to specialists
• Provide appropriate treatment for problems within scope of practice and refer appropriately when needed
• Utilize state‑of‑the‑art EPIC clinical and informatics technology to promote improved patient outcomes and streamlined continuity of care
• Access to dedicated support through the Office of Advanced Practice
• Established mentorship and orientation programs available for newly hired providers
• Unrestricted State of Connecticut Medical License required (or eligible)
• Minimum 2+ years of experience in a primary care or relevant outpatient setting required
