Qualified candidates must hold CT APRN/PA licensure, be a graduate of an accredited program, and have DEA/CT CS registration or be eligible to obtain both.

Qualifications

• Active Connecticut APRN or Physician Assistant license is required.
• DEA and CT controlled-substance registration required or eligible to obtain both.
• Graduate of an accredited Nurse Practitioner or Physician Assistant program is required.

Responsibilities

• Provide comprehensive primary and ambulatory care to a diverse patient population.
• Obtain medical histories, perform assessments, diagnose conditions, and interpret clinical findings.
• Develop and manage evidence-based treatment plans in accordance with established clinical standards.
• Educate patients and collaborate with physicians, nurses, behavioral health professionals, and other members of the care team.
• Maintain timely, accurate, and thorough clinical documentation within the electronic health record.
• Support organizational quality measures and continuity-of-care initiatives.
